Not asking the higher order questions could be just an absence of balance (his technigue no. 4). I think we as teachers might get comfortable with what works for us (lower vs higher, or higher vs lower). If we get good feedback like participation or test scores, we might intuitively stick to what works for us. Problem is, it may not work for all the students.
